Continuing Health Care (CHC)

About this service

Some people with long-term complex health needs may qualify for free health and social care arranged and funded solely by the NHS. This is known as NHS continuing healthcare (CHC).

Continuing healthcare is a package of care provided outside of hospital, such as in your own home or in a care home. 

It is a service for people aged 18 years and over who have significant ongoing healthcare needs. People who are eligible for continuing healthcare service may individuals who are reaching the end of their lives, or have long-term conditions as a result of a disability, accident or illness.

Who can use this service

To be eligible for NHS continuing healthcare, you must be assessed by a team of healthcare professionals (a multidisciplinary team). The team will look at all your care needs and relate them to:

  • what help you need
  • how complex your needs are
  • how intense your needs can be
  • how unpredictable they are, including any risks to your health if the right care is not provided at the right time
Your eligibility for NHS continuing healthcare depends on your assessed needs, and not on any particular diagnosis or condition. If your needs change then your eligibility for NHS continuing healthcare may change.
 
A decision about your eligibility for a full assessment for NHS continuing healthcare should usually be made within 28 days of an initial assessment or request for a full assessment.
 

If your assesment finds you are eligible for this service, NHS North East London, as your local Integrated Care Board (ICB), would be responsible for funding your full package of health and social care. 

This means that we will find suitable care to meet your assessed needs. We will always try to give options and choices that best match yours and your family’s preferences for the type and place of care provided.

If you are not eligible for NHS continuing healthcare, you can be referred to your local council who can discuss with you whether you may be eligible for support from them. Find out more on our Adult Social Care page.

If you still have some health needs then the NHS may pay for part of the package of support. This is sometimes known as a “joint package” of care.

How to access this service

In order to receive support from the NHS continuing health care service, individuals have to be assessed by NHS North East London to determine whether you are eligible.  

If you would like to have a continuing healthcare assessment for yourself, a family member or for a person you care for, you can speak to:

  • Your social worker (if applicable)
  • A member of the clinical team involved in your care
