Diabetes – Diabetes Prevention Programme (NDPP)

About this service

While type 1 diabetes cannot be prevented, type 2 diabetes is largely preventable through lifestyle changes. 

The Healthier You NHS Diabetes Prevention Programme, also known as the Healthier You programme, identifies people at risk of developing type 2 diabetes and refers them onto an evidence-based lifestyle change programme.

The Healthier You programme is available both as a face-to-face group service and as a digital service. When referred into the programme, people are free to choose between the two.

The free programme runs over 9 months and can help you reduce your risk of developing type 2 diabetes through healthy lifestyle changes.

Understanding your risk of type 2 diabetes

Find out if you’re at risk of developing type 2 diabetes by doing any of the following:

  • Answering a few simple questions on the Diabetes UK ‘Know Your Risk’ tool
  • Taking up the offer of a free NHS Health Check to assess your risk of type 2 diabetes, cardiovascular disease and other conditions such as high blood pressure (available for those aged 40-74)
  • Asking your GP Practice to check if you are at risk.

If you are at risk, you may be eligible for referral to the Healthier You NHS Diabetes Prevention Programme by your GP Practice.

Who can use this service

To be eligible to join this programme you must be at risk of developing type 2 diabetes, aged between 18 and 80 years old and:

  • Registered with a GP practice in City & Hackney, Newham, Tower Hamlets, Waltham Forest, Barking & Dagenham, Havering or Redbridge
  • Not pregnant
  • Have a blood sugar level (HbA1c) between 42 – 47.9 mmol/mol (6.0-6.4%) or a Fasting Plasma Glucose between 5.5-6.9 mmol/mol within the last 24 months
  • If you have a history of Gestational Diabetes (GDM), then you are eligible with a blood sugar level of  <42 mmol/mol or Fasting Plasma Glucose level of <5.5mmol/l.

How to access this service

If you meet the eligibility criteria you can get a referral through your GP practice. If you are unsure if you are eligible your GP will be able to check your risk and advise you.

If you have the following information you can self-register for this programme here.

  • NHS Number
  • Blood Test Result (either your HbA1c or FPG reading)
  • Date of Blood Test (must be within the past 12 months)
  • Your height & weight
