NHS Type 2 Diabetes Path to Remission Programme
The NHS Type 2 Diabetes Path to Remission Programme helps people living with Type 2 diabetes use a diet replacement approach, to lose over 10kg in weight, improve blood sugar levels, reduce the need for diabetes-related medication to achieve remission which means to achieve target blood sugar levels without medication.
What happens on the NHS Type 2 Diabetes Path to Remission Programme?
This is a free 12-month programme, funded by NHS England. It combines specialist nutrition, psychology and physical activity and throughout the programme. If you are eligible for this programme you will be offered free total diet replacement products including soups and shakes consisting of up to 900 calories a day for up to 3-months. During this time, you will be replacing all normal meals with these products. This 3-month period is followed by reintroducing food in a healthy and sustainable way and focusing on creating healthy new habits and maintain your weight.
This programme can be tailored for different dietary needs including vegan, vegetarian, halal and kosher.
The support you will receive is broken down below.
- Initial consultation with your coach to set goals and start the total diet replacement phase
- 12 weeks of a low-calorie diet using total diet replacement products, with personalised support from your coach with periods of self-led learning to achieve meaningful changes to your diet and lifestyle
- Change phase consisting of 4 weeks of food reintroduction, with coaching and self-led learning
- Sustain phase with monthly support from your coach to establish new healthy habits. Opportunity to do a refocus phase if you have gained weight. You will be able to continue self-tracking activities in the Oviva app
- Lifelong access to the learning resources and Oviva app to support healthier behaviours.

What is the eligibility criteria for this programme?
You must be:
- Registered with a GP practice in North East London
- Aged 18–65 years
- Confirmed diagnosis of Type 2 diabetes within the last 6 years
- Have a BMI over 27 kg/m2 (or over 25 kg/m2 if you are of Black, Asian or minority ethnic origin). To check your BMI click here.
- Motivated to change their lifestyle and can commit to the 12-month programme
You must not be:
- A current insulin user
- Pregnant or planning a pregnancy in the next 12 months
- Have had or are awaiting bariatric surgery (unless willing to come off waiting list)
How do I join the programme?
Because this programme is not suitable for everyone, referrals can only be made by a trained health care professional at your GP surgery. If you meet the eligibility criteria listed above and are motivated to join this programme please contact your GP surgery to discuss the next steps.
