The service is provided to residents of Barking and Dagenham, Havering and Redbridge
The community treatment team (CTT) works with adults in the community with an acute physical need who could potentially be treated at home, rather than in accident and emergency (A&E).
The aim of the service is to prevent unnecessary hospital admissions for those people who are experiencing an acute, physical health crisis, for example a suspected infection, falls without injuries, or an exacerbation of a long term condition such as COPD.
The service consists of doctors, nurses, health care assistants, occupational therapists, social workers and physiotherapists who work with primary care, community services and healthcare partners at Queen’s and King George’s Hospitals so that people can receive medical, nursing and therapy interventions in their own homes wherever possible, without need for hospital admission.
Aged 18 and over who have a non life-threatening, acute physical health condition that would ordinarily mean attending A&E.

Referrals to this service can be made by anyone. You can refer yourself or someone you care.
Referrals can also be made via GPs and healthcare professionals directly to the service.
