Your online Personal Health Record (PHR) is a collection of information about your medical history, including:
- details of test results
- appointments
- treatments
These records are important for:
- monitoring your health
- coordinating your care
- ensuring you receive the best possible treatment.
By having all your health information in one place, you can make informed decisions, easily communicate with healthcare providers, and manage your care more effectively.
Accessing Your Personal Health Information
Your personal health record can be accessed through the NHS App.
The app is connected to the Patients Know Best (PKB) and DrDoctor secure platforms which allow you to:
- manage your health information and test results
- access and share your medical history
- access your prescriptions
- access your care plans
- view and track appointments
- track symptoms and record measurements
- communicate securely with your healthcare provider
- reschedule visits
- manage your care
- submit forms
You can access these features by following the registration process on the NHS App.
