Support When It Matters (SWIM) is a Hackney-based, African, Caribbean and Dual Heritage (AC&DH)-led and run social enterprise offering support to people to maintain their wellbeing in the community. The aim is to work with AC&DH citizens who are registered at GP practices in four Primary Care Networks (PCNs) in City and Hackney who are experiencing complex needs. Patients can self-refer for this programme.
Patients can be referred from the following PCNs:
- Clissold Park
- Hackney Downs
- Springfield Park
- Woodberry Wetlands
