Neighbourhoods is continuing to bring local residents, community groups and services together within the PCNs to increase preventative partnership working at a local level. This is an exciting time as the Fuller Review on Primary Care Networks and their future within Integrated Care Systems has recommended that all PCNs should evolve into ‘integrated neighbourhood teams’ in a bid to improve services.
There is increasing evidence to suggest that working collaboratively across sectors and, in partnership with communities like Neighbourhoods, is key to improving health and reducing health inequality. The new website highlights how Neighbourhoods is already making a difference to people across City & Hackney.
