Social prescribers
Social prescribers can be helpful if you have are feeling lonely, isolated, or have a non-medical problem like bereavement that is affecting your health. They connect you to local services, groups and activities that can help you.
For example, this could include connecting you with local bereavement support groups, volunteering opportunities, arts activities, group learning, gardening, befriending, cookery, healthy eating advice, debt management, housing and sports.
Health and wellbeing coaches
If you need support with managing a long-term health condition such as Type 2 diabetes, with managing your weight or with your mental health, health and wellbeing coaches can give you the tools, motivation and confidence to take greater control of your life.
