Why Population Health Management matters and how our new Pathfinder Platform changes the game

Population Health Management (PHM) is a strategic way of looking at a whole community’s health, not just one person at a time.

Imagine a team of data experts, commissioners, public health teams and doctors using data to spot who might get sick before it happens and building a system that instead of waiting for people to show up at A&E or at their GP, reaches out to offer early support and help them stay well at home. It’s about joining the dots between GP surgeries, hospitals, social care, and community services to give people the right help at the right time.

Think of it as healthcare with a bird’s-eye view – seeing the big picture to make smarter, kinder decisions for everyone and facilitate the big shifts in the NHS 10 Year Plan – from treatment to prevention, from hospital to community and from analogue to digital.

This month, NHS North East London launches the Optum Pathfinder platform – a powerful new tool that brings PHM to life across the system. For the first time, we will have whole-system linked data together in a single analytics platform. That means GPs, analysts, public health teams, and system leaders can all access the same joined-up view of population health.

The platform is safe and secure and will be available across the NEL health and care system to help develop the most effective health interventions and systems of care. For the first time the data will be available at different levels – system-wide, borough or neighbourhood – and enable people to identify specific groups of people, such as those with long-term conditions or frequent service users, to offer targeted support.

Built-in dashboards and cost modelling tools will help track the impact of interventions, compare cohorts, and build a strong evidence base.

Local authorities and public health teams can use the data to plan programmes tailored to community needs. NHS Trusts and community providers can analyse service activity, capacity, and outcomes to improve efficiency and care quality. ICB system leaders and analysts can evaluate strategies and work to reduce health inequalities.

Integrated neighbourhood teams and multidisciplinary teams can use the tools to coordinate care across different services, ensuring patients receive joined-up support. GPs and Primary Care Networks can use the platform to identify patients at risk and provide more proactive care.

Nearly all our GP practices have signed up to a robust data-sharing framework and having it all in one place means that teams across the system no longer have to spend time and resource wrangling data or navigating IG requirements. Training has begun and the platform launches today 12 November.
